Nominations are now open for the following Department awards:
- Distinguished Service
- Disabled Veteran of the Year
- Appreciation
- Chapter Commander of the Year
- Chapter of the Year
- Large Business
- Small Business
- DVOP (Disabled Veteran Outreach Program)**see below
- LVER (Local Veteran Employment Representative)**see below
** The nominations for these awards generally come from the AZ Dept. of Economic Security, however if you work with a DES representative that you feel deserves recognition please feel free to send in the nomination.
Please submit nominations on the form that can be accessed from the link below. This is a fillable form and can be done online or you can print the form and fill it in on your own. If you choose to complete it offline please either type out the information or make sure the form is legible. The nominations need to be as detailed as possible so feel free to add pages as needed. Please return the form with supporting documentation to Department Headquarters NLT 1 APR 2012.
